Patient Rights and Responsibilities
To ensure proper care and to maintain the best possible cooperation, you have rights and duties as a patient and visitor, as do we as a hospital. As a patient at the Catharina Hospital, you can count on a correct approach and professional attitude from our employees and medical specialists. The employees and medical specialists expect the same correct attitude from you. If you feel unhappy, you can file a complaint. Those who do not comply with the rules of the house can be denied access to the hospital.
Your medical file
Our medical specialists maintain a medical record of all their patients. This file contains information about your state of health, examinations and treatments. Our employees safely store your file. Only healthcare practitioners who are directly involved in your treatment have access to the file. Healthcare providers have pledged confidentiality. Only with your written permission will and may they provide information to third parties.

Do your want to request your medical file? Download the authorization form here and print it out. Complete the authorization form clearly and completely. Sign the completed form and send it (without stamp) to: Catharina Ziekenhuis, Bureau Patientenbelangen, Antwoordnummer 298, 5600 VC Eindhoven.
You can also send the form by e-mail to Bureau Patientenbelangen: firstname.lastname@example.org. You will receive a copy by mail within 10 working days. Please note this term when making your (continued) appointments. You can only submit an emergency request in case of an emergency medical emergency. You can note that on the authorization form. You can also note if you do not want to receive the file by mail, but you want to retrieve it personally. Since your files contain detailed personal information files are not send all across the world. We do this to minimize the risk of fraud. We ask you to get a mail adres in the Netherlands, or make use of a dutch contact person which we can mail the file to.
