What to bring to our hospital?
Your first visit to the outpatient clinic
Upon your first appointment with us you will need to bring the following:
- Referral form from your general practitioner or primary care physician
- Valid identification (passport, ID card or driver’s license)
- Valid insurance card with your policy number
- All medications you are taking (in original packaging)
- Patient pass that you will obtain before your appointment
For your follow up appointments you will need to bring:
- Your patient pass
- The appointment card that you will receive at your first appointment
When you’re going to stay on a nursing department one or more days:
- Patient pass
- All medications you are taking (in original packaging)
- Emergency contact information
- Toiletries
- Clothing
If you are scheduled for surgery, please do not wear any jewelry, this includes wedding rings. We advice you to wear loose-fitting comfortable clothing.
Please leave all valuables at home where possible. The Catharina Hospital is not responsible for theft or damage to personal property.
